As you prepare for your appointment, please bring the following items.
ARRIVING FOR YOUR APPOINTMENT
Dr. Morgan and Dr. Tolentino's office is in Christie Clinic in Bloomington on Empire located at 2502 E Empire St, Ste C, Bloomington. You will park on the east side of the building (nearest the airport) and enter on the 1st floor (see entrance below). Christie Clinic is located on the 2nd floor; stairs and an elevator are located inside the double doors.
