Effective April 1, 2023 Medical Hills Internists transitioned to Christie Clinic at Medical Hills.


WHAT TO EXPECT:

  • FIRST VISIT:
    Upon arrival to your first visit you may be asked to sign a new consent form and provide your current insurance information.

  • SERVICE PROVIDED:
    Christie Clinic at Medical Hills will continue to offer all services previously available to you, including medical visits, laboratory, basic imaging and physical therapy services.

  • INSURANCE:
    There is limited change in the insurance plans accepted as we move to Christie Clinic at Medical Hills. A list of accepted insurance plans is provided on the back of this letter; however, it is encouraged that you confirm coverage directly with your plan.

  • MEDICAL RECORDS:
    Your historical medical records will be available for your provider to access as you continue your care with Christie Clinic at Medical Hills.
